15 is one of the many minigames that you can find in your Trioptimum Entertainment Pak in System Shock.
This minigame is available only in the CD-ROM versions of the game (both CD and updated floppy version that came along with it on the disc).
The program can be found on the Executive Level in the viewing room behind the back rows (looks like an Audio Log).
Puzzle pictures include: numbers 1-15 (16 is left out), small animated picture of Edward Diego, animated SHODAN, Abe Ghiran, and small TriOptimum logo with transparent background.
